Cannulation of central venous vessels is a common practice in medicine, both for diagnostic and therapeutic purposes; traditional techniques employ blind access, with reference to anatomical landmarks. Even cannulation of arteries and peripheral veins is usually performed with blind techniques, but it can be very difficult with not visible/palpable vessels.

Ultrasound support for vascular access entails some advantages compared to traditional blind techniques:

– successful placement of intravascular catheters in nearly all cases, with a reduction in procedural time and cost.
– a reduction of central venous catheter placement-related mechanical complications (arterial puncture, hematoma, pneumothorax, hemothorax);
– immediate recognition at the patient’s bedside of the most dangerous mechanical complications (arterial hemorrhage, pneumothorax, hemothorax), with the possibility of guiding specific treatment;
– immediate in-procedure control of the catheter tip position within the vessels, by direct visualization and contrast ultrasound.

The aim of the course is to impart knowledge and skills on the use of ultrasound in accessing the vascular system, for the placement of central and peripheral vein catheters or other devices.

Target Audience

Medical professionals regularly engaged in intra-venous catheter placement and intravascular device procedures. These includes mainly anesthetists, intensivists, emergentists, critical care physicians, nephrologists, internists, cardiologists, pre-hospital healthcare workers.

Course Modules

  1. Introduction and Classroom Agreement: Course introduction and establishment of classroom guidelines.
  2. Ultrasound Physics and Technology (optional): Understanding the fundamentals of ultrasound physics and technology as applied to vascular access.
  3. Medical-Legal Considerations and Issues: Exploring the medical-legal aspects related to vascular access procedures.
  4. Semiotics and Ultrasound Technique for Peripheral and Central Venous Vessels: Study of the signs and ultrasound techniques for both peripheral and central venous vessels.
  5. Ultrasound Approach to Peripheral and Central Vessel: In-depth examination of ultrasound-based approaches for both peripheral and central vessels.
  6. Eco Support for Peripheral and Central Access: General Principles and Site Selection: Discussion of the principles and general guidelines for selecting cannulation sites in peripheral and central access with echo support.
  7. Seldinger Technique and Material Selection: Detailed explanation of the Seldinger technique and the choice of materials for vascular access procedures.
  8. Post-Procedure Check-up and Positioning: Procedures for post-procedure assessment and patient positioning.
  9. Complications: Identifying and managing complications associated with vascular access procedures.
  10. Video (or demonstration): A Complete Procedure Visual demonstration of a comprehensive vascular access procedure.
  11. Out-of-Plane Eco-Guided Access: Instruction and practice in out-of-plane echo-guided needle access techniques.
  12. Oblique Ultrasound-Guided Access In Plane: Skill development in oblique ultrasound-guided needle access within the scan plane.
  13. Longitudinal Ultrasound-Guided Access In Plane: Training in longitudinal ultrasound-guided needle access within the scan plane.
  14. Final Evaluation: Assessment of course participants to gauge their knowledge and skills.
  15. Post-Course Paths and Conclusions: Discussion of future learning paths and a conclusion to the course.
